Not sure if this is true. I am pretty sure it's 3 shots, and the person getting the kill must get it within four seconds of the assistants last shot (almost like getting a multi kill). That is why they are so hard to get.

Not sure about TU, but I think Bungie said that it was 40% when Reach was released.

You need to take at least 40%, and they have to die within 5 seconds after your last shot.
